PESA / TESA and TESE/micro TESE are great treatment options for men who have do not have any sperms in their semen i.e. are azoospermic because of a block in their vas deferens.
In them the sperms are retrieved by aspiration from either the head of epididymis (PESA) or from the testis (TESA) or from taking a small testicular biopsy (TESE) or via micro TESE.
This procedure is done under local anesthesia and is not painful.
A very fine needle is used for aspiration, and the aspirate is examined under the microscope for sperms. The sperms are then used for injecting into the egg as in ICSI.
Intracytoplasmic sperm injection (ICSI) technique has been greatest boon for a male with very low sperm count to father his biological child. The steps involved are as follows
Ovarian Stimulation: Hormonal injections are used for stimulating the woman's ovaries. The injections are timed according to her period dates. Her response to the injections is judged by Trans-vaginal sonography. When they reach the mature stage, the eggs are retrieved by aspirating the fluid from the follicles.
Egg retrieval is done through the vagina under general anesthesia and sonography guidance.
ICSI (Intra cytoplasmic sperm injection): On a special microscope with a micromanipulator, a single sperm is picked up in fine micro needle and injected manually into the oocyte.
Embryo Transfer: Embryos are transferred back into the uterus of patient either on Day 3 or Day 5.
The extra embryo can be cryopreserved for further use.
The couple can expect a 30-40% chance of conception per cycle.
The Success rate of PESE/TESA/microTESE with ICSI treatment at our center is 30-40%
